α-Ketoisocaproic acid 是一种亮氨酸代谢物，已知可诱导胰腺 β 细胞分泌胰岛素。发现α-酮异己酸酯的肠内输注可改善内毒素血症。从其转化中得到的酮体可用作能源。 α-酮异己酸的积累以支链酮酸尿症为特征，这是由于缺乏支链 L--酮酸脱氢酶活性而引起的。 α-Ketoisocaproic acid 在氧化磷酸化反应中引起解离，并作为 α-酮戊二酸脱氢酶的代谢抑制剂。因此，导致在支链酮酸尿症中观察到的线粒体稳态缺陷。
